Position Summary:

Capernaum Place is a 20 unit transitional housing complex structured with 6 months leases and a maximum residency period of 2 years. It is the objective that all guests successfully transition into permanent housing in that time frame in accordance with their unique Individual Service Plans. The case manager provides advocacy and case management services to an assigned caseload ensuring adherence to each client’s individual service plan; The Housing specialist will also provide housing search workshops and supportive services to promote resident self-sufficiency, integration in the community and permanency in housing.

Position Description:

Housing Specialist

  • Assessing housing barriers of families experiencing homelessness and determine housing and service needs.

  • Apply knowledge of residential lease contracts and educate clients of their rights and responsibilities.

  • Seek new housing opportunities and housing lotteries that Guests may qualify for.

  • Attend housing appeals and assist Guests with the housing appeals process.

  • Educate our Guests on the differences between Section 8, subsidized housing, and market rate.

  • Act as a liaison between Guests, the Ministry, and various community housing programs.

  • Fulfills other position-related duties as required.

Case Manager

  • Ensure the continuing mission of Lazarus House of maintaining a high standard of care by treating all who seek services with dignity and respect.

  • Assist with the development of individual service plans.

  • Provide crisis intervention, safety planning and information about domestic violence programs and resources

  • Along with your team, proactively identify potential challenges as a tool to promote a safe, supportive, respectful home for residents.

  • Meet with clients at least once per week for advocacy, problem resolution, and ensure compliance with individual service plan.

  • Along with the program director, interview prospective Capernaum Place candidates to assess fitness for admission to the program.

  • Leads and co-leads workshops.

  • Meets weekly with the Capernaum Place team to review status of caseload.

  • Maintain records and progress notes for each resident.

  • Collaborate and share stories with our communications team.

  • Conduct drug screens, monthly apartment inspections, house meetings.

  • Work with families to develop a housing procurement, financial and self-sufficiency plan that they will then continue working on with their case manager.

  • Assist in the development and encourage adherence to a personal budget through pro-active housing and budget counseling sessions.

  • Represent LHM at community meetings and other relevant functions.

  • Share on-call duties.

  • Fulfills other position-related duties as required.

Qualifications:

  • Bilingual, English/Spanish required.

  • Demonstrate strong commitment to the mission of Lazarus House Ministries

  • Associate’s degree in human services, psychology or related field.

  • Knowledge and understanding of tenant’s rights and responsibilities as well as strength based case management.

  • Strong knowledge of housing search techniques.

  • Knowledge of state and federal laws that affect and protect the poor and homeless populations.

  • Knowledge and ability to set and maintain boundaries with staff and Guests.

  • Strong organizational skills and a high level of attention to detail.

  • Demonstrated knowledge of community resources, social services agencies, and land lord tenancy rights.

  • Ability to adapt to all crisis situations as they arise.

  • Ability to deal tactfully and establish and maintain harmonious working relationships with others.

  • Strong verbal and written communication skills.

  • Proficient in Microsoft Office Suite.

Physical Demands/Working Conditions:

The information described here includes but is not necessarily limited to general physical requirements, demands and/or working conditions that an employee is likely to encounter at Lazarus House, Inc.   Additional requirements may arise as your employment and needs of the Ministry evolve. You must be able to handle or possess the ability to meet the following:

  • Prolonged sitting and/or standing, some bending, stooping and stretching.

  • Eye-hand coordination and manual dexterity sufficient to operate equipment pertinent to your area- office, kitchen, warehouse, cleaning, etc.

  • Occasionally lift, move and carry up to 50 pounds.

  • Travel from building to building within the Ministry and/or to external agencies.

  • Cross-cultural sensitivity and ability to work with or be exposed to diverse populations, including people who are homeless or have mental illness or other challenges

  • Interact effectively with staff, guests, volunteers, benefactors and the general public.

  • Manage multiple priorities

  • Able to manage the stress of the job.

  • This position requires the individual to offer physical stability and support to the guests of the residence, such as but not limited to lifting and assistance if a guest was to fall, stabilizing guests during walking, assistance in execution of stairs, and helping guests up and down from chairs.
